Requesting an Immunization Record

Requesting an immunization record is easy:

• You can obtain your immunization records through the Docket web browser or by downloading the Docket app. Docket is an application that is securely updated with information from the North Dakota Immunization Information System, North Dakota Immunization Information System.
• Our Immunization Record Request Form can be completed and submitted online.
